  • All sacramental record requests must be made through the parish at which the sacrament was administered.

    If you do not know the name of the parish where you received a sacrament, but have done your due diligence in trying to locate the record, we can assist you in locating this record. Please complete the form below to submit your request.

    In order to protect the privacy of the individual involved, certificates are only issued to the person whose record is requested or to a person who provides proof of parental authority, guardianship, power of attorney, executor/personal representative or other legal status on behalf of the person whose record is requested. No official certificates are issued for genealogical purposes. Photo ID must be presented.

  • All baptismal certificates are issued by the parishes. If you know the name of the parish, you must have made an effort to contact the parish office before your request will be processed by Archives.

  • All first communion certificates are issued by the parishes. If you know the name of the parish, you must have made an effort to contact the parish office before your request will be processed by Archives.

  • Additional sacraments after baptism are noted on a person's baptismal certificate. If you do not know the name of the church where you received additional sacraments after baptism, it is recommended that you get a newly issued baptismal certificate in order to see if it is noted on your baptismal record.

  • All confirmation certificates are issued by the parishes. If you know the name of the parish, you must have made an effort to contact the parish office before your request will be processed by Archives.

  • Additional sacraments after baptism are noted on a person's baptismal certificate. If you do not know the name of the church where you received additional sacraments after baptism, it is recommended that you get a newly issued baptismal certificate in order to see if it is noted on your baptismal record.
