Sacramental Record Request Form

All sacramental record requests must be made through the parish at which the sacrament was received.

If you do not know the name of the parish where you received a sacrament (Baptism, First Communion, Confirmation, etc.), the Archives can assist you in attempting to locate the record. Please complete the form below to submit your request.

In order to protect the privacy of the individual involved, certificates are only issued to the person whose record is requested or to a person who provides proof of parental authority, guardianship, power of attorney, executor/personal representative or other legal status on behalf of the person whose record is requested. No official certificates are issued for genealogical purposes. Photo ID must be presented.
